{"id":2345,"date":"2019-11-04T09:47:54","date_gmt":"2019-11-04T09:47:54","guid":{"rendered":"http:\/\/www.harmonihomes.com\/cy\/?p=2345"},"modified":"2021-08-31T11:47:20","modified_gmt":"2021-08-31T10:47:20","slug":"healthyhome-service-launched","status":"publish","type":"post","link":"https:\/\/www.harmonihomes.com\/cy\/blog\/healthyhome-service-launched\/","title":{"rendered":"Healthy@Home service launched"},"content":{"rendered":"<p>Healthy@Home is a dedicated short-term service to help people who have been in hospital long term return home safely.<\/p>\n<p>The service also helps to keep people that could be facing hospital admittance but would be better at home, stay at home.<\/p>\n<p>Taking referrals from healthcare professionals the dedicated Healthy@Home facilitators will work with patients\/prospective patients to identify the blocks to them returning or remaining at home.<\/p>\n<p>Healthy@Home provides:<\/p>\n<ul>\n<li>A flexible 7-day- a-week service available across Cardiff and the Vale<\/li>\n<li>A dedicated phone line for easy referrals from 8.30am-4.30pm every day<\/li>\n<li>Voicemail and email inboxes for out of hours referrals<\/li>\n<\/ul>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter size-medium wp-image-2342\" src=\"http:\/\/www.harmonihomes.com\/cy\/wp-content\/uploads\/sites\/13\/2019\/11\/Healthy@Home-Logo-300x300.jpg\" alt=\"\" width=\"300\" height=\"300\" srcset=\"https:\/\/www.harmonihomes.com\/cy\/wp-content\/uploads\/sites\/14\/2019\/11\/Healthy@Home-Logo-300x300.jpg 300w, https:\/\/www.harmonihomes.com\/cy\/wp-content\/uploads\/sites\/14\/2019\/11\/Healthy@Home-Logo-150x150.jpg 150w, https:\/\/www.harmonihomes.com\/cy\/wp-content\/uploads\/sites\/14\/2019\/11\/Healthy@Home-Logo-768x768.jpg 768w, https:\/\/www.harmonihomes.com\/cy\/wp-content\/uploads\/sites\/14\/2019\/11\/Healthy@Home-Logo-1024x1024.jpg 1024w, https:\/\/www.harmonihomes.com\/cy\/wp-content\/uploads\/sites\/14\/2019\/11\/Healthy@Home-Logo.jpg 1181w\" sizes=\"auto, (max-width: 300px) 100vw, 300px\" \/><\/p>\n<p>Commissioned by Cardiff and Vale Health Board, the service works with health professionals to meet the needs of the person being discharged or about to be admitted to hospital, agreeing a first visit within three days of discharge.<\/p>\n<p>It provides a light-touch support to build confidence and independence in line with Prudent Healthcare and assessment of the patient\u2019s needs.<\/p>\n<p>Healthy@Home supports people with:<\/p>\n<ul>\n<li>Ensuring they are eating properly. This might include taking them shopping or setting up deliveries as well as helping with food preparation<\/li>\n<li>Collecting prescriptions and setting up deliveries to help them manage their medication needs independently<\/li>\n<li>Breaking down daily living tasks into manageable steps, supporting them to do what they can and accessing help to make tasks easier<\/li>\n<li>Getting out and about, building confidence and providing emotional support<\/li>\n<li>Basic home safety checks to help them feel safe at home as well as preventing falls and other health risks<\/li>\n<li>Connecting with the most effective local sources of support, whether this is statutory or third sector<\/li>\n<li>Attending appointments and completing paperwork<\/li>\n<li>Making a success of returning or remaining at home, preventing admittance or re-admittance to hospital<\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>Healthy@Home is a dedicated short-term service to help people who have been in hospital long term return home safely.
