After three years of delivery, we have released an Impact Statement about Wellbeing 4U to celebrate the achievements of the service and our patients.<\/h4>\n

Access to the right kind of support to help people to live happily and in good health in our communities is really important to us.<\/p>\n

Wellbeing 4U is a community wellbeing service delivered by Thrive, the team that manage specialist accommodation and related services for United Welsh.<\/p>\n

The service was commissioned by Cardiff and Vale University Health Board in 2016 and since then, it has been life-changing for patients and a life-line for health professionals.<\/p>\n

Using a social prescribing model, Wellbeing 4U supports people to be responsible for their own health by placing them at the centre of their care, promoting independence and personal responsibility.<\/p>\n

In the first three years of service, the team have supported over 3,800 patients and produced approximate cost savings of over \u00a3370,000.<\/p>\n
